On your first visit to our office, you will fill out a personal and medical information form. This information is confidential and will only be provided to others with your permission.

The information requested will help us in your care. Your past medical history may play an important role in the treatment we prescribe and the manner in which it is rendered.

A full report of our findings and treatment will be sent to your referring doctor. Feel free to ask us anything regarding your treatment or diagnosis. We will provide you with this information as completely and clearly as possible.

You may preregister with our office by filling out our secure online Patient Registration Form. After you have completed the form, please make sure to press the Submit button at the bottom to automatically send us your information. On your first visit to our office, we will have your completed form available for your signature. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.

Please Note:

Our online forms use the Adobe Acrobat 5 Plugin to allow patients the convenience of completing their health history and registration forms from home or work. Please download the free plugin from Adobe's web site if it is not already installed on your system. It is important that you have at least version 5 of the plugin, in order to successfully use our forms.

Our office hours are 8:00 am - 5:00 pm on Monday, Tuesday, Wednesday, Thursday, and Friday. The Dana Point office is closed on Tuesday; the Lake Forest office is closed on Wednesday.

We know that our patients' time is valuable. In order to better evaluate your needs, we recommend an initial consultation appointment. At this time the doctors can review your medical history and the proposed treatment. You will have a chance to ask questions and better prepare yourself for the surgery appointment. In some circumstances, the consultation and surgery may be performed at the same appointment. If you have an urgent problem, we'll accommodate you as soon as possible - usually the same day.

Our doctors are often called upon to render emergency treatment, which may produce a delay in the appointment schedule. Be assured that if you need emergency treatment, you too will be seen as quickly as possible.

Our practice can only serve our patients well if its financial policy is clearly stated. Our fees are based on the type and extent of treatment you require. We prefer that you pay for our office services at the time the services are rendered. Delay in payment increases clerical and administrative costs, forcing us to reflect these increases in our fees.

Insurance companies do not set our fees, but they do establish a list of fees that they refer to as "usual and customary". The insurance company will reimburse you a percentage of those fees.

Our fee is your responsibility regardless of how much your insurance covers. Before surgery, it is a good idea to read your policy or check with your insurance company representative concerning your coverage. Our financial coordinator is always available to help you take maximum advantage of your insurance. In appropriate cases, our office will be glad to submit a pretreatment estimate to better delineate the actual dollar amount your insurance company will provide. This usually takes 4-6 weeks to complete.

If you need to make other financial arrangements, please feel free to talk with our financial coordinator prior to treatment.

Please bring your insurance forms with you to your first visit.
